Medication Therapy Management (MTM)

Medication-related problems are a significant health concern. It is estimated more than 1.5 million preventable medication-related problems occur in the United States each year.

Kerr Health’s PharmD clinicians will review all medications, including prescriptions, over-the-counter products and herbal supplements, identifying potential problems and alerting a physician if a change is recommended.

Having completed APhA-certificate training in the area of MTM, our pharmacists conduct face-to-face MTM sessions with patients by physician referral, for several NC employer groups, as well as Outcomes Pharmaceutical Health Care, Mirixa, and Humana. Additionally, we provide telephonic MTM in more than 15 other states. We are very proud of being recognized for our performance from a variety of groups.

ChecKmeds NC

Kerr Health pharmacists have been active in the ChecKmeds NC program since its debut in October 2007. Established by the NC Health and Wellness Trust Fund Commission, the primary goal of ChecKmeds NC is for all North Carolinians (65 years of age or older with a Medicare Part D prescription insurance plan) to meet with a pharmacist, to evaluate all their medications — including prescriptions, non-prescriptions, herbal therapies and vitamins.

During this comprehensive medication review, also known as a CMR, the pharmacist may be able to identify cost-saving opportunities, drug-drug interactions, therapeutic duplications, incorrect administration technique, non-adherence to therapy and untreated diseases or conditions. Following the CMR, the pharmacist shares his/her findings with the patient’s primary care provider(s) if a change is recommended.

Kerr Health pharmacists have performed more than 11,000 CMRs since the program began in 2007.
