Chronic Kidney Disease

Chronic Kidney Disease affects most body systems and functions such as the production of red blood cells, blood pressure control, vitamin D levels, and bone health.  Patients will often experience symptoms including (but not limited to): nausea, fatigue, itching/dry skin, headaches, unexpected weight loss, and appetite loss.  We understand the complications of battling Chronic Kidney Disease and we are here to assist with managing all side effects.

Personal Service

Our commitment to individual care and comprehensive services drives our approach to patients dealing with complex health conditions.

Each patient is assigned a personal Patient Care Advocate (PCA).  Your PCA serves as a point of contact for our services.  They will be in touch regularly to ensure your needs are being met.  Your PCA will call to find out if your delivery arrived as expected, if you are having issues with your medications, and to remind you when your prescription is due to be filled.

Clinical Care

Our Clinical Pharmacists are available to speak with you about your medications and answer any questions you may have.  Pharmacists and Nurses are available 24/7 to assist with questions or issues as they arise.  We are committed to ensuring that individuals with Chronic Kidney Disease have access to quality care.
