Diabetes Education
Annually, more than 600,000 people are diagnosed with diabetes in the U.S., and another 300,000 show symptoms of pre-diabetes. They are all at risk of heart attacks, strokes and kidney problems.
The Kerr Health Clinical Diabetes Program is offered to physician practices and helps contribute to patient self-management. An informed, active patient allows doctors to spend more time focusing on care and treatment. The program also provides for evidence-based decision support to improve quality of care.
Disease Self-Management Education (DSME)
Our DSME program, which is recognized by the American Diabetes Association, allows for personalized diabetes education that is delivered in a variety of teaching styles and methods.
